Skilled Nursing

Our dedicated group of nurses provide a variety of duties based on the individualized care plan that is created during your initial visit by the Registered Nurse. Our team works closely with your Physician to coordinate any needed medical care during your time with us. Some of these services include:

  • Medication Management
  • IV infusion/PICC Line
  • Wound and/or Ostomy Care
  • Pre/Post-Operative Care
  • Diabetic management/injections
  • Catheter Care
  • Nutritional Therapy: G-tube/J-tube
  • Patient/Family Health Education
  • Mobile lab services
Physical Therapy

Physical therapists are licensed professionals that develop, maintain, and restore maximum movement and functional ability. Our physical therapists will perform a comprehensive assessment of the individual. The therapist will then create an exercise and treatment program with specific goals that are unique to each patient.

Dietitian

The registered dietician has training and knowledge to initiate and manage nutritional support. This can include enteral (tube) feedings, as well as parental nutrition (intravenous nutrition). The dietician can also assist with training and education to the patient and caregiver(s).

Social Worker

Medical social workers assess the social and emotional factors of the patient in the home setting. The social worker is available to provide psychological support and coping techniques. The social worker will also help assist the patient and family with any needed community resources.

Speech Therapy

Speech- Language Pathologists are experienced in helping patients with difficulties that sometimes occur as a result of an illness, injury, or surgical procedures. The speech therapist will work towards rehabilitating a patients ability to communicate, improve cognitive skills, and ensure safe swallowing to reduce the risk of medical complications.

Occupational Therapy

Occupational therapists are licensed professionals that help people of all ages who have physical, sensory, or cognitive problems. The therapist will focus on improving the patients ability to perform activities of daily living. These activities include bathing/showering, eating and feeding oneself, toileting, and personal hygiene.

Home Health Aides

Our certified home health aides work under the supervision of the registered nurse and can provide the following services:

  • Personal care; Bathing, Grooming, Dressing
  • Assisting patient to bed, chair, or commode
  • Turning and positioning patients
  • Exercise
  • Change bed linens
  • Meal preparation
